ACA Reporting Form 1095-B

As required by the Affordable Care Act (ACA), United Healthcare, on behalf of the National Plan and UPREHS, recently mailed the required Form 1095-B to Union Pacific agreement employees, retirees, and former employees.  These forms were not sent to members of the Union Pacific Railroad Employes Heath Systems (UPREHS).  Forms will be mailed to UPREHS members the end of January.  Members will receive two forms - one with employee information only, and one form with dependent information (if applicable).

In addition, United Healthcare has advised us the following information was inaccurate on the forms sent to employees covered by the National Plan:

1. Box 8 reflects code B (for employer sponsored coverage) instead of code E (for multi-employer plan)
2. Incorrect EIN (employer identification number)
3. Incorrect Plan address
4. Incorrect phone contact number.  However, the cover letter included with the form does reflect the correct phone number for Railroad Enrollment Services, a group within United Healthcare that administers the National Plan.  If someone calls the incorrect number, they will be routed to Railroad Enrollment Services at (800) 753-2692.

While unfortunate, the incorrect information is not critical for income tax filing purposes.  The information needed, provided in Part IV of theform, regarding health care coverage is correct.  As such, the forms will not be reprinted and mailed.
